A: Access your basic medical record in seconds through the St. Mary’s General MyChart-Patient Portal, which will provide you information such as lab results, prescribed medications, discharge instructions, and more. If you require a copy of your complete medical record, please visit Obtaining My Medical Record or contact our Medical Records Department at 973-365-4306

Q: How can I get a copy of my birth certificate?

A: The Hospital is able to provide birth certificates for newborns birthed at St. Mary’s General within five (5) days of birth. After five days, all requests for birth certificates must be directed to:

City of Passaic Office of Vital Statistics, at 973-365-5602, located at Passaic City Hall, 330 Passaic Street, Passaic, NJ 07055.

Q: How may I obtain copy of family members’ death certificate?

A: The Hospital does not provide death certificates. Death certificates may be provided by the funeral home that is facilitating services for the deceased. After the services are complete, all requests for death certificates must be directed to:
